In a nutshell: This article describes a study which resulted in measureable reductions in unplanned hospitalisation for a cohort of patients with complex health issues and demonstrates the potential to significantly improve patient care particularly for patients at the end of their lives. Advance care plans are used to identify patients wishes if their health deteriorates.
