In a nutshell: Representatives from the Royal Cornwall Hospitals NHS Trust, Cornwall Partnership NHS Foundation Trust, Cornwall Hospice Care, South Western Ambulance Service NHS Foundation Trust and representatives from independent community care providers have formed and helped to develop a bespoke learning path to guide and support staff working in primary, secondary and social care who provide end of life care. The “passport” launched in collaboration with REACH Cornwall- which supports skills development – will allow its holders to collect evidence of all their learning.
